Peoplechart Corporation in collaboration with the American College of Cardiology ACC) designed
and developed the CardioSmart Health Tracker (tool) to enable patients and physicians to work together
in the management of hypertension based on ACC-approved clinical guidelines. The basic version of the
tool can store daily blood pressure measures (averaged and trended), capture information about patient's
health-related activities, remind patients to track their blood pressure, and produce reports that can
be shared with their physicians in the next office visit. For an upgraded version, Peoplechart combines
the best practice cardio-care protocols with features that encourage direct physician use and timely
physician-patient engagement.
