Pricing Plans

Individual Plan ($24.95/person, per month)

In this plan, you receive all the benefits described in the section Products and Services above. In particular, a membership with Peoplechart provides you with our Premiere Collection Service, where Peoplechart does the work of requesting and retrieving copies of your medical records from up to 6 different physicians or healthcare organizations. Copies of the information are subjected to our one-of-a-kind and state-of-the-art electronic indexing, tagging, and categorization (by page and by groups of pages). When done, you can easily find and sort pages by any combination of these categories through your Peoplechart account:

1. Name of the provider from whom we collected your medical records
2. Author of the content on the page
3. Date authored
4. Type of medical record (over 80 categories)
5. Medical specialty

By using a pre-defined organizational schema, you can choose to view only certain types of medical records, save them in a folder, or automatically compile them into a printable report. You can also choose to search by one or multiple criteria to narrow results or simply view the information organized by sections in any of our three standard reports.

Family Plan ($64.95/family, per month)

In the Family Plan, you can register up to 4 individuals in your family as members. Based on 4 individuals, the monthly fee would only be $16.25 per person. Each member receives all of the benefits described in the Products and Services section /listed above), some of which are detailed in the Individual Plan directly above.

As head of household, you can choose to register yourself or leave the membership open for another individual in your family. To qualify for the Family Plan, registrant must be an immediate family member (father, mother, and/or dependents — children under 18 years)
