Peoplechart's Mission and Philosophy
“Our mission is to improve the quality and coordination of care by providing individuals, family members, and
healthcare providers with an integrated solution for managing a lifetime of health information. We believe that by
enabling individuals and organizations with the tools and services to take an active role in the management of patient
information, they can be effective and engaged members of their care team.”
– Megan Mok, Founder and CEO
Team effort means individuals and health care team can communicate and exchange information with each other.
You will find that the Peoplechart solution can be used by an entire team of health professionals, because, if desired, an entire
client management system can be layered on top of the member application. From disease management and prevention standpoint, this
program-level application makes facilitating care and communications with patients and with each other that much easier.
Guiding the product features is a sophisticated platform of roles and privileges that help protect patient privacy and systems
security.
Not everyone needs to have access to every feature or every piece of information about the patient. At the helm, the patient
or an authorized health professional decides which information is available for access (or kept private) by each user assigned to
the 'care community'. The patient can override any user in their care community by enabling or disabling feature privileges or by
editing the time duration for account access.
Underlying sophistication, we remain realistic in our approach.
We believe that many patients and health professionals are making the effort to embrace technology but as of now,
not everyone is computer ready or fully converted to industry-accepted standards for information sharing (exchange).
At times, the telephone or fax machine may be the fastest way of communicating information from one place to another.
Peoplechart Collection and 24×7 Customer Support services can help an individual or health organization…
- Collect copies of the medical records from various physicians
- Securely distribute them anytime day or night
Being realistic also means being able to learn.
Towards this goal, we offer a complete and illustrated resource, A.D.A.M. Health Encyclopedia, where you can find certified
information about specific conditions, diseases, or medications, and more!
