1. Administer
In a few easy steps, organizations can create health programs for different departments or facilities by using our
systems, without compromise to patient privacy. Program managers can quickly create or edit roles/titles, set privileges,
or assign specific staff to patients or clients. They can easily communicate with each other, change staff assignments,
and switch primary to secondary (backup) team so every patient is being cared for in a thoughtful way. We even provide a
way for you to track and adjust for differences in time zones for care that is being transitioned to other regions of this
country or around the world.
2. Communicate
We developed an internal messaging system so that people, tasks, and schedules can be coordinated among
staff users, patient, physicians, or caregivers. The messaging system is much like an email system with editorial
features and can be used to communicate between individuals or among groups. Unlike emails, the messages are
sent/received inside of one secure server and guided by the roles and privileges assigned to each user.
Email notifications are sent to the recipient to check the Inbox any time a message is sent. Users can attach
specific reports, indexed documents, and health profiles onto the message to highlight certain information about
the patient.
3. Facilitate
In addition to communications, staff can facilitate the delivery of care in several ways:
- Invite physicians and other care team members to join as new users of the Care Community, where medical information
can be shared, patient referrals made, medical notes added, and the support structure for a specific patient grows.
- Assist by recording information about the patient’s health and history that you can update or edit according
to the needs of your organization.
- Search for and highlight medical records or reports via Manage Folders and refer patient to other physicians via
Manage Referrals.
- Guide patients to use the certified Health Encyclopedia when they have questions about their disease, conditions,
and the latest treatments.
4. Integrate
Our integration engine is the life blood to accurate and complete information about the patient. The application electronically
pulls from the source, maps the data in standardized formats, and stores it in the PHR along with other information about the patient.

A major part of Peoplechart's service is focused on capturing a complete and accurate picture of patient’s health and
health history. We believe that by assisting in the process of medical record collection and when possible, electronic
capture from various provider sources, we have added value to the completeness and accuracy of the information about the
patient. An equally important priority for our systems development team is to ensure that the information is organized
in ways that enable the user to find the information as needed. The final requirement is to provide flexibility — add
means for the user to enhance the PHR through notation or by uploading data files onto the PHR.
- Medical Records Collection — A health manager, patient, or a patient-authorized care giver can submit a
request for Medical Records Collection service. Once the provider’s information is entered, our collection service performs
several communications with the provider office to retrieve the medical records. Peoplechart team has extensive experience in
medical record retrieval and a deep understanding of national and state laws governing privacy, timely delivery of the medical
records, and provider fees. When it comes time, we send email notifications to the health manager and patient to given an update
on our efforts and when it comes time to collect new pages from the same provider.

- Electronic Transmittal and Data Mapping — Peoplechart can capture, translate, and populate the database with electronic
data — with diagnostic labs for now and e-meds to follow. The tedious work of mapping data from one format to another is already tackled
by Peoplechart — and can save valuable time and development resources for any organization. Users can download electronic data files in
XML, CSV, Excel, PDF, TIFF, and HTML format.

- Document Upload — Any user who is authorized to upload documents (as a feature privilege) can upload any electronic
file documents from the Personal Computer onto the web-based PHR. Through a wizard, the user can type in a file name, date, and set
the privacy flag on any uploaded document. The file, when clicked open, is displayed in a separate browser that can read over 20
different file formats including Word, Excel, PowerPoint, PDF, and HTML.
- Web-Based Forms — Over twenty e-forms tailored for the patient and for the health care professionals to add information
about the patient’s health.
Patient Forms
Patients and their caregivers can use one of 20 sections to add health and health history to Peoplechart.
Physician Forms
Healthcare professionals can provide physical exam notes, progress notes, consultations as well as treatment and care plans.
